The wellbeing and healthcare of our residents at Eastbourne Grange Care home is at the core of our services, outlining principals to ensure every resident has access to high quality healthcare that meets their needs.
In the UK over 405,000 older people (65+) currently live in care home, this represents 16% of older people above the age of 85 according to studies. The average care home resident has multiple long-term conditions, functional dependancy and frailties, of which 75-80% have cognitive impairment. Residents are likely to have an increased quality of life and health condition at Eastbourne under our care if health services reflect their needs personally on a case by case basis.
Our staff are trained to deal with many health conditions and place long-term effective plans in place, however we recognise that sometimes specialist external support maybe required, we support residents to continue with their local GP if local to us or we assign local GP’s for specialists when required. Identifying early changes in condition, diet or behaviour can be key when providing treatment. Health needs are complex, late stage conditions can provide further complications and therefore person-centred models are key in building trust and knowing your resident to spot changes and instigate the appropriate action when needed. Providing this access and advocating its uses is welcomed by us, the residents welfare and health is key to live their best life.
We understand the right culture and environment is essential in providing care for our residents, one that is combined with both support and clinical governance. Our staff, led by our home manager Kandy Walster states, ‘’Reliability and consistent access to health professionals, such us our local GP’s helps to build trust early with residents, building reviews with both residents and their families ensures our care is of the highest quality. Individual based lines of communications with healthcare advisors and loved ones also helps to reduce anxiety and build for a more sustainable quality of life”.
The complexities and nature of needs for all elderly patients with frailty means that often there is no set rule or method in which one should commission external health services. Therefore, understanding your resident, building trust and care strategies which evolve is paramount. Regular reviews, records and care plans are mandatory as well as nutrition and state of mind in conjunction with behavioural changes. We couple all this with in depth integration periods from both the families, loved ones, existing medical health professionals and existing GP’s.
